Caregiver Award
Hartford Funeral Home would like to extend our most sincere respect and appreciation to caregivers who serve our community. Nursing and Rehab facility professionals (physicians, nurses, chaplains, social workers, CNAs, healthcare administrative professionals, or volunteers) are the professionals who serve our community selflessly and help during a time of grief for the families they serve.
In an effort to recognize these dedicated people for their selfless devotion and generosity of heart, we have created our "Caregiver Award" program. Each quarter we will honor a caregiver based upon a review of nominations received from peers, friends, patients and their families, and others who feel a particular person excels in her or his profession. We will honor one outstanding caregiver each quarter with a gift card and an award.
Please use this form to nominate and recognize an important caregiver, so we can honor that person through recognition and a special gift to show our tremendous appreciation.
